Forest Health Group offer a range of online services including ordering repeat prescriptions, booking appointments and viewing parts of your medical records. PATIENT ACCESS is a secure website that will enable you to access these services.
How do I register for patient access?
You must be a fully registered patient, and need a letter from us containing personalised access codes. To get this letter you can complete an Online Registration Form and then you will need to come to Reception with photo ID e.g passport/driving licence. Alternately you can visit Reception and ask for a PATIENT ACCESS registration form or download one below. Please bring the completed registration form into the surgery. Your access codes and registration instructions will be emailed to you within 3 working days. Once you have the access codes and instructions you can connect to the web site at any time and register.
